Telemonitoring

The Health Team, in collaboration with a technological partner, offers a clinical telemonitoring solution, which allows the remote monitoring of a group of patients, in the comfort of their home, ensuring the monitoring of a set of clinical parameters obtained automatically on a daily basis.
This solution is qualified under the framework agreement for telemonitoring services promoted by the Shared Services of the Ministry of Health.

The solution includes the following set of equipment: Tensiometer, Oximeter, Thermometer, Glucometer, Pedometer, Scale and Electrocardiograph.

The Health Team provides first-line assistance for any issue related to this solution, training in the use of equipment for the patient and caregiver, the daily monitoring of parameters, as well as follow-up visits, based on pre-defined criteria.
